R-0447 Leopard-Skin Pill-Box Hat - edit (2:20, as opposed to 3:50 for album version)
The B-side was Most Likely You Go Your Way And I'll Go Mine, unusually the early mono mix from the Canadian mono album release of Blonde On Blonde, see International Mono Releases.
Columbia 4-44069 was separately released in Canada in 1967, again without a picture sleeve. For pictures of the generic sleeves used for this single, see 7" Single Sleeves.

I originally reported that the UK and Netherlands versions of this single had the unedited album version of the song, but I'm now informed that the UK release had the R-0447 edit too, so it's listed here. Thanks to Robert Kornovich and Roger Ford for confirming this is actually a new mix of the song as well as an edit. The promo release has white CBS labels, the commercial release has orange CBS labels, both came in the same picture sleeve, which is identical front and back.

This Norwegian release has the same catalogue number as the West German/Dutch/Italian/Danish release above, but the sleeve only lists Most Likely. The sleeve is a folded single sheet with the reverse printed upside down. The record still has Leopard-Skin Pill-Box Hat as the A-side and was also sold in Denmark inside the Danish sleeve illustrated in the entry above. This single was not released in Sweden.
